Abstract

Sleep disorders are common in the general population and can lead to significant morbidity and mortality. They cause or exacerbate preexisting medical and psychiatric conditions and are associated with high rates of depression, anxiety, and impaired daytime functioning. They may also lead to poor occupational performance, motor vehicle accidents, cardiovascular and endocrine disorders. CPAP is the main therapy of SRBD. It eliminates of the sleep fragmentation, reduces restless sleep, headache and day time sleepiness.
The aim of the current work was to describe the personal characteristics of patients with obstructive sleep apnea (OSA), classify the severity levels of OSA using the AHI, determine prevalence of different comorbidities among studied patients since time of diagnosis, compare the comorbidities and associated risk factors among patients with severe and extreme OSA with those having sleep apnea of lesser severity (mild and moderate levels) and determine the percentage of the patient adherent to positive pressure therapy.
This study enrolled 244 patients previously diagnosed as SRBD using full night polysomnography. All patients were subjected to history taking, clinical examination and several questionnaires (including STOP-BANG questionnaire and Epworth sleepiness scale (ESS) and BREF QOL score).
Fifty three percentage of our studied patients were females, whereas 47% were males. The mean age of studied patients was 56.9 ±. 12.36 years. Age of onset of SRBD was 46.11 ± 11.98. Age was statistically higher among female patients and among severity group 2 (p=0.002,. 014 respectively ). Among our female patients, 58% were post menopause.
The mean of BMI was 42.6 kg/m2 ±. 5.9 kg/m2. Epworth sleepiness scale had a mean value of 17.9 ±4.8.
For purpose of comparison, patients were categorized into two groups according to OSAHS Severity (Group1:Mild + moderate) included 93 patients (38.1%) and (group 2:Severe+ Very severe+ Extreme) included 151 patients (61.9%).
Comorbidities were highly prevalence among our studied patients (91%).This is high ratio compared to similar studies, justified by the fact that we dealt with patients referred to tertiary referral hospital. Obesity and hypertension were the most common co-morbidities. Obesity (BMI ≥30) was present in 203 patients (85.3%) and HTN was present in 166 patients (68%).
Other comorbidities were in descending order as follows: type 2 DM, fatty liver, bronchial asthma, dyslipidemia, COPD, CHF, IHD, DVT, pulmonary embolism, hypothyroidism, CKD, AF, stroke, infertility, cancer in (50%, 43%, 20%, 19.6%, 19%, 15.8%, 11.9%, 11.5%, 9%, 7.3%, 6%, 5%, 2.6%, 2%, 1%) of our patients, respectively.
Regarding comparing comorbidities which were significantly more common among severity group 2 were Obesity, hypertension, DM, CHF, DVT, PE, hypothyroidism, (p=. 0.005,0. 004,0.006,0. 001,0.032,0. 003 and 0. 030 ) respectively.
