Abstract The objective of this study was to conduct a systematic review to compare the oncological and functional results of trans-oral laser microsurgery (TLM) versus open partial laryngectomy (OPL) in treatment of T1, T2 glottic cancer larynx. Methods: Search was conducted in the Medline database (www.pubmed.com) using the keywords laser, Open partial laryngectomy, treatment T1 T2 early glottic cancer larynx. The study was limited to articles published in English language, conducted on human subjects in the last 30 years. Evaluation Method: Articles reviewed by the authors and data compiled in tables. Results: No randomized controlled trial was identified. Six studies were included as they compared both the oncological and functional outcomes, no significant differences were identified between TLM and OPL with respect to local control disease specific survival (DSS) or disease free survival (DFS). TLM showed better results than OPL in relations to the functional outcomes. Conclusion: Current treatment modalities (TLM and OPL) for T1 and T2 glottic cancer larynx treatment, although showed good oncological and functional outcomes, comparison of both modalities were based on low-level evidence studies. TLM should be preferred as it was accompanied with good oncological and functional outcomes with less morbidity and mortality. OPL should be maintained to special cases where TLM is contraindicated or there is no availability of the machine or the experience. |
