Abstract

The experience of psychosis is difficult and distressing, to both patients and care givers, difficulties arise from not only the experience of psychotic symptoms, but also the associated emotional and cognitive dysfunction which significantly affect the quality of life.
Impaired sexual function is evident prior to the first episode of psychosis in people who are yet to receive antipsychotic medication and in a similar degree to that seen in people with a first psychotic episode, and is also related to the severity of psychotic symptoms.
This study was carried out at Dermatology , Venereology and Andrology Department of Assiut University Hospital , Egypt, to assess the erectile dysfunction and measure serum prolactin and total testosterone level in patients with first episode psychosis in the period from June 2017 through June 2018.it was carried out on 40 first episode psychotic patients and 40 age matched control group, the mean age was (33.83 ± 9.5) years.
The patients were asked about duration of the disease , history suggestive of any psychotic symptoms , and were asked to measure serum prolactin and total testosterone level ,erectile dysfunction was evaluated by Arabic version of international index of erectile function-5 (IIEF-5) .
The patients were diagnosed according to DSM-5 into 6 (15%) patients with Brief Psychotic Disorder, 11 (27.5%) patients with 1st Episode Mania with Psychotic Features, 23 (57.5%) patients with Schizophreniform Disorder with the mean duration of the disease was ( 3.49 ± 0.3) months.
The study showed significant erectile dysfunction in FEP patients with increase in serum prolactin level and decrease in total testosterone level .
There was significant negative correlation between disease duration and IIEF-5 score , but positive correlation with prolactin level .The study showed more ED with increase in prolactin level .
Patients with schizophreniform disorder & 1st Episode mania with psychotic features had lower total testosterone level than brief psychotic disorder .
Conclusion:
from results of the present study the following points could be concluded:
• Men with first episode psychosis are at risk for development of erectile dysfunction.
• Erectile dysfunction can put a high psychological burden on first episode psychosis patients.
• Increase duration of untreated psychosis lead to more erectile dysfunction and hyperprolactinemia .
• Increase in prolactin level lead to more erectile dysfunction.
