Abstract

Summary
Osteochondral lesion of the talus (OLT) is a broad term describing an injury or abnormality of the talar articular cartilage and adjacent bone.
The ankle is one of the most frequently injured joints in sporting activities and in the general population. The incidence of misdiagnosis or delayed diagnosis of OLTs in a group of patients with unexplained chronic ankle pain has been reported to be as high as 81%.
There is no consensus concerning the etiology of OCD lesions. Traumatic insult is widely accepted as the most important etiologic factor of an OLT.
osteochondral lesion of the talar dome will most commonly presented with a complaint of ankle pain mostly with prolonged weight bearing and high impact activity as running, sometimes poorly localized and nonspecific.
MRI proved very sensitive and specific and allows good visualization of the articular surface. It may actually overestimate the size of the lesion because it is very sensitive to bone marrow. While CT cannot assess the articular cartilage directly, it is more effective than MRI at evaluating the bony defect.
The average success rate of non-operative treatment is 45%.
The operative treatment of the OLT is individualized according to patient factors and OLT factors.
