Abstract

Postcoital bleeding refers to spotting or bleeding that occurs after intercourse and is not related to menstruation. There are multiple etiologies for this common complaint in which most are benign such as cervicitis or cervical polyps. However, the most serious cause of postcoital bleeding is cervical cancer.VIA has demonstrated high sensitivity for detecting cervical cancer but is limited by low specificity. VIA requires minimal resources and training. It has also been recommended by WHO, as an alternative to cytology to pick up a patient at risk for cancer cervix.The purpose of this study is to study the role of the Visual Inspection with Acetic acid (VIA) test in cases with post coital bleeding.This study done on 62 women within the reproductive age with post-coital bleeding coming to the outpatient gynecologic clinic from Mansoura University main hospital and Kafr Sad central hospital. In the present study there were 25 of cases were +ve with Visual Inspection with Acetic acid (VIA) and 37 of them were -ve with Visual Inspection with Acetic acid (VIA).This study showed that a statistically significant difference between two groups regarding gravidity, parity, vaginal discharge and contraception and it shows non-statistically significant difference regarding menarche, date of 1st intercourse and time of last labor.In the present study, VIA test positive cases were 25 case, the majority of these cases were chronic nonspecific cervicitis which represent (12.9%) and the following were chronic nonspecific endocervicities with nabothian cyst (6.45%) endocervical polyp (4.8%), papillary endocervicities (3.2%), endocervicities with inflammatory atypia (3.2%) endocervicities with metaplasia and cervical ectopy (3.2%) endocervicities with microglandular hyperplasia (1.6%) and LSIL 4.8%.VIA test negative cases were 37 case: vaginitis was the most common cause 35.4% then displaced IUD 9.6% and utrine prolapse 8.1%., genital warts 4.8%, thrombocytopenia 1.6% were the minor causes .<Conclusion: VIA test is a mandatory step in cases of post coital vaginal bleeding to pick up the hidden cervical precancerous lesions before treatment. Colposcopy may be an essential step in cases of positive suspicious VIA test to take Colposcopic guided punch biopsy.
