Abstract

Summary
Laparoscopic techniques became one of the highly used techniques in the management of acute appendicitis .
Our study indicated that using laparoscopic techniques in management of acute appendicitis has many advantages that have been studied in many other hospital settings. The rapid return to resumption of diet, decrease the length of stay in the hospital, faster return to normal activity and rapid post-operative recovery after the surgery were the main findings in our study and were in the favor of laparoscopic techniques. On the contrary, the duration of the surgery was shorter in the open appendectomy group. Based on the previous advantages of Laparoscopic over open appendectomy, we argued that laparoscopic technique for appendectomy is at least as acceptable and in effect as its open counterpart. More frequent practice of this procedure may lead to saving more time for the patients. Lastly, It should be continued as a standard option for patients presented with acute appendicitis.
Fourty patients had been divided into two groups: open appendectomy (OA) group (20 case) and laparoscopic appendectomy (LA) group (20 case).
All patients who had been admitted for appendectomy had been assigned to the study. The diagnosis was made clinically according to Alvarado score (Alvarado 1986). Patients with appendicitis included in the study had been performed at Beni-Suef University, all patients included were at 16 years of age or older.
A total of 40 cases were eligible for participation in the study period, 20 cases under went open appendectomy (mean age= 22.55+ 5.30) and 20 cases under went laparoscopic appendectomy (mean age= 22.05+ 4.46). the number of males were 8 cases, 7 cases underwent open appendectomy and only one case for laparoscopic appendectomy. The majority of patients’ participants were dwelling Urban areas 29 (72.5%) and only 11 (27.5%) cases were from rural setting.
Out of the total participants, eight cases showed up intra operative complication, six case in the Las group and two cases in the OAs group. Further than four cases (20%) which need conversion from laparoscopic to open technique, all other cases were successfully operated laparoscopically
There was no significant differences in the mean age in both groups (P value > 0.05). Mean operative time was 82 (range: 40-180) minutes in LA group as compared to 70 (range: 30-120) minutes in OA group, the difference was statistically significant (P value < 0.001).
Mean operative time was 33.95+6. minutes in OA group as compared to 61.10+18.99 minutes in OA group, the difference was statistically significant (P value < 0.000), the time required until return to fluids and soft diet was 14.05+6.21 and 7.80+2.82 in the OAs and LAs groups respectively and the p value was significant (p value= 0.000).
The mean days of stay in the hospital (2.03+0.80 OAs, 1.05+0.32 LAs) and the mean days until return to work and normal life activities (13.30+1.17 OAs, 10..40+2.16LAs ) were both significant at (P value < 0.000
