Abstract

Testicular dysfunction is the most common side effect of cytotoxic chemotherapy cyclophosphamide (CP), which is a commonly used drug as anticancer and immunosuppressive agent during organ transplantation. Stem cells and lipoic acid are detected in the human and animal reproductive system, hence assumed to play a useful role in the reproductive cells.
The present study aimed at assessing the possible effect of mesenchymal stem cells (MSCs) therapy on CP-induced testicular injury in albino rat, compared to that of lipoic acid (LA).
37 adult male albino rats weighing 150-200g were divided into 2 groups:
group A (Control group): 10 rats, each intraperitoneally (IP) by injected 10 ml/kg/body weight saline for 17 weeks.
group B (experimental group): 27 rats were IP injected by endoxan 15 ml/kg body weight dissolved in saline every other day for 6 weeks. Two rats were sacrified to confirm testicular injury. Then, the rest of animals were subdivided into 5 subgroups (5 animals per group) as following:
• SubgroupB1 (Model subgroup): Rats were IP injected with endoxan at a dose of 15 ml/kg/body weight dissolved in saline every other day for 11 weeks.
• Subgroup B2 (Stem cells subgroup): Rats received endoxan as in subgroup B1 and single dose of MSCs (1x106 per animal) injected into the tail vein after 6 weeks from beginning of the experiment.
• SubgroupB3 (LA subgroup): Rats received endoxan as in subgroup B1 and LA ampoules administered (IP) at a dose of 10 mg ⁄ kg ⁄ every other day dissolved in saline for 11 weeks.
• Subgroup B4 (Stem cells plus LA subgroup): Rats received endoxan as in subgroup B1+ MSCs as in subgroup B2+ LA as in subgroup B3.
• Subgroup B5 (recovery subgroup): Rats were left without any drug treatment for 11 weeks.
At the end of experiment, all rats were sacrificed, and testes were processed for paraffin sections. A histological study was carried out using H&E and Masson’s trichrome stain. Immunohistochemical study using the BCL2 antiapoptotic onchoprotein and a morphometric study were also conducted.
The results in subgroups B1 and B5 showed a significant distortion in seminiferous tubules, degeneration of spermatogenic cells, perivascular and intertubular fibrosis, and negative immunoexpression of BCL2. But the use of MSCs and LA in subgroups (B2, B3 and B4) conferred improvement against all of the above-mentioned changes. But the improvement in subgroup B2 (Stem cells treated) was better than that in subgroup B3 (LA treated). Meanwhile, combined treatment with both MSCs and LA in subgroup B4 showed better improvement more than using each one of them singly.
Conclusion
Administration of CP to rats induced testicular lesions. Withdrawal of the drug did not ameliorate this effect. The concomitant administration of MSCs and LA with CP led to an improvement in testicular lesions induced by CP. Meanwhile, combined treatment with MSCs plus LA resulted in better improvement more than using each one of them singly.
