Abstract

There are different surgical approaches for treatment of chronic periodontitis patients with deep periodontal pockets without osseous resection and with minimal soft tissue resection including flap curettage, OFD and MWF. It was proven that MWF is a predictable approach for treatment. The adjunct application of HA enhances the probability to obtain good healing of the periodontal tissue following surgery. Moreover, HA helps to decrease the post-surgical complications such as: pain, edema and bleeding.
The aim of this RCT was to assess the effect of topical HA on postoperative pain after MWF versus MWF alone in patients with chronic periodontitis.
Forty patients with chronic periodontitis having PPD ≥ 5 mm and CAL≥ 5 mm were randomly assigned to be treated with MWF surgery in conjunction with either 0.2% hyaluronan gel (test) or placebo gel (control) application for 1 week, following initial nonsurgical periodontal therapy and re-evaluation after 4 weeks. Patient discomfort measured by VAS and post-operative consumption of acetaminophen analgesic were evaluated daily for 1 week. Tissue edema, tissue color values were recorded at 1 week post-surgical. CAL, PPD, PI, and BOP values were taken at baseline and 2 months.
Test group results revealed that serial VAS measurements showed significant decrease in the 7 VAS measurements after 1 week postsurgical (p = 0.0075).
