Abstract

Obstructive sleep apnea (OSA) is defined by full or partial occlusion of the upper airway during sleep. The resulting occlusions restrict airflow and can produce repeated oxyhemoglobin desaturation, sleep fragmentation, or both.We aim to find out an alternative method rather than manual titration to prescribe CPAP for OSAS patients as manual titration is labour intensive, costly and delays access to treatment due to long waiting list. Methods: Forty CPAP-naïve patients with moderate to severe OSAS (AHI ≥15 per hour) were studied but only 30 cases were available for final analysis. The patients were allocated to standard manual CPAP titration, partially attended APAP titration for 3 nights and CPAP prediction using Miljeteig & Hoffstein., 1993 and Sériès., 2000 equations.Results: Statistically significant difference was found between autotitrated pressure and manual titration pressure with higher manual pressure (13.6 ± 2.4 vs 14.9 ± 3.1 cmH2O, p value = 0.019) , also between Séries equations-based pressure and manual titration pressure with higher manual pressure (11.1 ± 1.3 vs 14.9 ± 3.1 cmH2O, p value = 0.000) and between Hoffstein equations-based pressure and manual titration pressure with higher manual pressure (8.90 ± 1.4 vs 14.9 ± 3.1 cmH2O, P value = 0.000 ). Statistically significant difference was found when comparing Séries equation-based pressure and autotitrated pressure with higher autotitrated pressure (11.1 ± 1.3 vs 13.6 ± 2.4 cmH2O, p value = 0.000) and between Hoffstein equation-based pressure versus autotitrated pressure with higher autotitrated pressure (8.90 ± 1.4 vs 13.6 ± 2.4 cmH2O, p value = 0.000).Conclusions: Manually titrated CPAP can be predicted from autotitrated pressure. Consequently, manual titration could be spared for selected cases (e.g. when APAP is contraindicated) so this can decrease cost, long waiting lists and effort needed for manual titration. APAP titrated pressure can also be predicted from Hoffstein equation in case of non-availability of APAP device.
