Abstract

Aim: To compare the effect of ball and locator attachments on 15 degrees distally inclined canine implants assisting mandibular complete overdenture regarding the metabolic activity and implant stability. Materials and Methods: Six completely edentulous male patients of mean age 60 years were selected for this study from the clinic of prothodontic department. For all patients two15 degrees distally inclined implants (13 mm length and 3.5 mm width ) were inserted bilaterally in the canine areas .After three monthes of implant insertion, the implants were exposed and according to the type of attachments, the patients were divided into two equal groups : group A: Patients received two ball attachments screwed into angled overdenture abutments (15 degrees) and group B: Patients received two locator attachments screwed into angled overdenture abutments(15 degrees). Implant stability and peri implant metabolic activity( levels of glycoseaminoglycans , condriotin 4 sulphate in the peri- implant sulcular fluids) were evaluated immediately (T0), 3 monthes (T3) and 6 monthes (T6) after pick up of female metal housings attachments and overdenture delivery. Results: The comparison between the means of PTVs in the two groups regarding each interval showed insignificant difference at T0, T3 and T6 although the increasedimplant stability in group B more than in group A. When comparing between the means of PTVs at different time intervals in each group , significant increase in implant stability was observed from ( T0- T3), (T0-T6) and insignificant increase from (T3- T6).There was a statistically in significant difference for GAG and C4S levels in the peri implant sulcular fluid at T0, T3 and T6 between the two groups. Within the same group, there was a significant difference in GAGS and C4S between T0 and T3, between T0 and T6 and between T3 and T6. Conclusion: In accordance to the results of this study the following conclusions can be done :- Although the insignificant difference between the used attachments ,the locator attachments can be considered more favorable than the ball attachments regarding the preservation of 15 distally inclined implants used for assisting mandibular complete over denture.
