Abstract

Tonsillectomy is a common surgical procedure in children andcauses trauma to the oropharynx, with subsequent postoperative pain,variety of methods have been attemptedwith the aim of to reducing the pain associated with tonsillectomy, the use of analgesics has been proven to decrease pain after operation also the risks related to severe painsuch as bleeding and dehydration due to poor oral intake. This study was conducted in Otorhinolaryngology Head and Neck Surgery department, Tanta University Hospital from first of April 2018 till the First of April 2019. 150 patients were included in this study as 3 groups of patients , chosen from our outpatient clinic indicated for operation aged 3-7 years after excluding patient with severe cardiac, renal, hepatic, blood diseases or any co-morbidities. The aim of study is to compare the effect of use of analgesic drugs after tonsillectomy operation in children for better control of pain after this painful operation. Patientswere given paracetamol , ibuprofen or diclofenac in 3 groups of patients asa syrup form in uniformed bottles, given by the nurse of hospital and instructed to be given three times daily for 10 days and follow up of patients at days 3rd ,7th and 10th day after operation in outpatient clinic . 49 Pain and behavioral change scores were obtained from parents using the Parents Postoperative Pain Measure scorewhich is a 15- item questionnaire so total scores range from 0 to 15[66-68] The outcome measures besides pain score were the number of attacks of post-operative bleeding or admission in the hospital for severe pain or dehydration. The results showed significant pain control (lower pain scores) of ibuprofen and diclofenac over paracetamol at 3rd and 7th postoperative day but show decrease pain postoperatively at 10thday with no difference between 3 drugs ( lower pain score for the 3 drugs ) During course of treatment: 2 cases develop post-tonsillectomy bleeding (1 case ibuprofen and 1 case diclofenac groups) and 4 cases admitted to hospital for severe pain and dehydration ( 3 cases paracetamol , 1 case ibuprofen and 1 case diclofenac groups), these cases were treated conservatively with no other complications . So, ibuprofen and diclofenac drugs showed marked decrease of pain level especially at 3rd and 7th day after operation as compared to paracetamol after operation
