Abstract

Background: Mechanical ventilation (MV) has been used throughout history as a basic tool in the treatment of patients with respiratory failure, and as a means of improving prognosis. Almost 40% of mechanically ventilated patients have difficulties during MV weaning, due to multiple factors.
Objectives: To Asses Diaphragm Atrophy in mechanically ventilated pediatric patients. To asses the effect and the impact of Diaphragmatic Atrophy in weaning of mechanically ventilated patients.
Patients and Methods: This cohort study was conducted from October 2018 till July 2019 in the pediatric intensive care unit (PICU) of the children Hospital of Ain Shams University, Cairo, Egypt. The ethical committee of Ain Shams University approved the study and a written informed consent was obtained from the parents. The study was conducted on 30 children with acute respiratory failure requiring respiratory support in the form of invasive mechanical ventilation admitted in Ain shams university pediatric hospital PICU.
Results: 19 (63.3%) patients were successfully weaned and liberated from mechanical ventilation while 11 (36.7%) patients failed. 22 (73.3 %) patients were discharged from ICU while 8 patients (26.7%) died during ICU admission. Diaphragmatic atrophy was assessed using bedside US examination done shortly after intubation within 36 hours and prior to extubation which yielded two measurements for each patient.
Conclusion: Diaphragmatic atrophy does occur in mechanically ventilated pediatric population and affects patients weaning outcome and fate.
