Abstract

Psoriasis is a common, relapsing, chronic, disfiguring, inflammatory and proliferative condition of the skin, in which both genetic and environmental factors have a role. The disease is variable in duration, periodicity of flare and extent. Clinical variants are common as it involves nails, scalp, mucosae, and joints. Psoriasiform dermatoses are lesions either clinically or histologically mimic psoriasis such as: seborrheic dermatitis, pityriasis rubra pilaris, lichen simplex chronicus, pityriasis rosea, pityriasis lichenoides and mycosis fungoides.The aim of this study is to correlate between dermoscopic features of psoriasis and some psoriasiform dermatoses to their histopathological features.This study included (60) patients diagnosed as psoriasis (30 cases) or with psoriasis as a one of the differential diagnoses (30 cases). They were (27) males and (33) females. Dermoscopy was done for all of cases and the diagnosis was confirmed by histopathological examination.The demographic data and the clinical examination showed that age and the clinical signs of psoriasis showed statistically significant difference between the two groups.This study showed that the light red background, diffuse distribution of the white scales and regular homogenous distribution of vessels were the most common dermoscopic features of psoriasis. Also, the study revealed statistically significant agreement between the dermoscopic and histopathological findings of these findings.This study showed that orange yellowish structureless area, white scales, dark brown globules, focally distributed dotted vessels and linear irregular vessels are the most frequent dermoscopic findings of pityriasis lichenoides chronica. This study showed that there is an agreement between the dermoscopic and histopathological finding in pityriasis lichenoides chronica as orange yellowish structureless area and extravasated RBCs, hemosiderin deposition.This study also showed that the yellowish background, the peripheral distribution of the white scales and the patchy distribution of the vessels were the most common dermoscopic features of cases with pityriasis rosea. The dermoscope revealed statistically significant agreement as compared to histopathological examination in detecting each of superficial perivascular lymphocytic infiltrate, dilated dermal blood vessels and extravasated RBCs.This study showed that whitish plugs, yellowish background, linear and dotted vessels are the most frequent dermoscopic findings of pityriasis rubra pilaris. This study showed that there is a good agreement between the dermoscopic and histopathological findings in pityriasis rubra pilaris as linear-dotted vessels and dilated dermal vessels, whitish plugs and follicular plugging.This study showed that the dull red background and both patchy distribution of vessels and diffuse distribution of the scales were the most common dermoscopic features of eczema cases. Also, the study revealed statistically significant agreement between the dermoscopic and histopathological findings in detecting dilated capillaries within elongated dermal papillae, parakeratosis and irregular hyperplastic rete ridges.
