Abstract Our study was done after obtaining Assuit University Hospital Ethical Committee approval and informed written consent from the patients admitted in Assuit University Hospital in the department of orthopedics who were planned for total knee replacement surgery in the period from December, 2016 till February, 2019.Our study included 60 patients, aged between 18-60 years old. Patients were randomized into two Groups: group B (18ml of 0.25% Bupivacaine + 2ml saline) group Bf (18ml of 0.25% Bupivacaine + 1mic/kg fentanyl diluted with saline to reach total volume 2ml The current study showed that preemptive addition of 1mic/kg fentanyl to 18ml of 0.25% Bupivacaine to reach total volume 20ml for femoral nerve block guided with ultrasound for total knee replacement surgery on side of operation resulted in a reduction of VAS pain score over the postoperative 24 h, prolonged the duration of the block, longer time till first analgesic requirement, more patients satisfaction, less incidence of nausea and vomiting. As seen in our results that the VAS (Visual Analogue Scale) was significantly low starting from 4 Hrs, 8 Hrs, 12 Hrs, 16 Hrs In the current study no local anesthetic toxicity, no hematoma or excessive tissue trauma had been developed at the site of injection in both group this result could probably due the guidance of ultrasonography that enabled better visualization of the femoral triangle before injection. Conclusion The addition of fentanyl to bupivacaine in femoral nerve block guided by ultrasonography improve the post-operative pain in patient subjected to total knee replacement surgery. |
