Abstract

Traumatic ocular injuries are common ED presentations worldwide, and diagnosis could be postponed due to concurrent injuries.
We conducted a study evaluating patients with probable traumatic ocular injury who were recruited at Emergency department, Alexandria main university hospital, Egypt. During this study 60 patients (62 eyes) with probable traumatic ocular injury were included. Each case was assessed by an emergency clinician with POCUS using a linear transducer (7.5 MHz), via an ophthalmologist who did complete ocular examination and they were assessed by a radiologist after orbital CT imaging. Demographic data
and achieved results were tabulated. Specificity, Sensitivity and Negative, positive predictive values and accuracy were calculated subsequently.
In all measures of accuracy, diagnosis by POCUS compared with ophthalmologist clinical examination for diagnosis of retinal detachment, vitreous detachment and vitreous hemorrhage also we compared between emergency ocular ultrasonography and computed tomography for diagnosis of IOFO in cases with traumatic ocular injuries.
For retinal detachment, sensitivity and Specificity of eye ultrasound in comparison to ophthalmologic examination was 75.00% and 100.00% respectively with a positive and negative predictive value of 100% and 98.31% respectively with accuracy of 98.39%.
For vitreous detachment, the sensitivity and Specificity of eye ultrasound in comparison to ophthalmologic examination was 100.00% and 98.33 % respectively with a positive and negative predictive value of 66.67% and 100.00 % respectively with accuracy of 98.39%.
For vitreous hemorrhage, the sensitivity and Specificity of eye ultrasound in comparison to ophthalmologic examination was 75.00% and 98.28 % respectively with a positive and negative predictive value of 75.00% and 98.28 % respectively with accuracy of 96.77%.
For globe foreign body, the sensitivity and Specificity of eye ultrasound in comparison to CT of the orbit was 100.00% and 98.33 % respectively with a positive and negative predictive value of 66.67% and 100.00 % respectively with accuracy of 98.39%.
So POCUS may allow correct detection of traumatic ocular injuries in cases with facial and head trauma.
