Abstract

Although Portland cement has been recognized as the reference material for conservative pulp vitality treatments in histopathological studies, it has some disadvantage as a long setting time (2h 45m hours), need of hydration during setting, and difficult handling characteristics, lack of radiopacity and may contain heavy metals. Therefore, re-searchers continue searching for materials with improved physical properties, adequate sealability, and antimicrobial action similar to that of calcium hydroxide. Biodentine has antimicrobial, antifungal, and anti-inflammatory properties, so the current study evaluates using Biodentine as alternative to Portland cement
This study was conducted to evaluate the histopathological response of healthy dog pulp to direct pulp capping with Biodentine compared to Portland cement.
The study was done on 3 apparently healthy adult Mongrel dogs, Maxillary and mandibular left and right first, second, third, and fourth premolar and first and second molar were utilized in each dog. Split mouth technique was used where teeth on the right side of the dog’s jaw (12 teeth) were capped with Biodentine, while those on the left side (12 teeth) were capped with Portland cement. Three dogs were sacrificed after one, two and 3 months respectively and the teeth were removed from the surrounding bone and prepared for histopathological examination (H&E) and histochemical evaluation (Masson trichrome stains).
Histopathological evaluation, at the first evaluation period (one month after capping of pulp with tested materials) the pulp response to both material was the same, in both groups 83.3% of the specimens showed absence of inflammation response limited to pulpotomized site (score 0), while 16.7% of the specimens showed mild inflammation at pulpotomized site (score 1).
At the secound evaluation period (two months after capping of pulp with tested materials) the pulp response to both material was the same, in both groups 75%% of the specimens showed absence of inflammation response limited to pulpotomized site (score 0), while 25%% of the specimens showed mild inflammation at pulpotomized site( score 1).
At third month of follow up period 100% of specimens capped with Biodentine showed absence of inflammation cell at the exposed pulp (grade 0). In Portland cement 75% of specimens showed absence of inflammatory reaction (grade 0) while 25% specimens showed mild inflammation (grade 1).
Hard tissue deposition, after one month of pulpotomy procedure by Biodentine 25% showed absence of hard tissue formation (grade 0) and 75% showed mild of hard tissue formation (grade 1). While in Portland cement 100% showed absence of hard tissue formation (grade 0).
After two months of pulpotomy procedure 16.7% specimens in Biodentine specimens showed mild formation of hard tissue (grade 1) and 83.3% specimens showed moderate formation of hard tissue (grade 2). While in Portland cement 25% specimens showed absence of tissue formation (grade 0), 75% specimens showed mild formation of hard tissue formation (grade 1).
After three months of capping procedure 16.7% in specimens in Biodentine showed mild formation of hard tissue (grade 1) and 83.3% showed sever formation of hard tissue (grade 3). While in Portland cement 25% specimens showed mild formation of hard tissue (grade 1), 75% specimens showed moderate formation of hard tissue (grade 2)
Histochemical evaluation, after one month of pulpotomy procedure by Biodentine 25%of the specimens showed no fibrosis (score 0) and 75% showed mild fibrosis (score 1). While in Portland cement group 100% specimens showed no fibrosis (score 0).
After two months of pulpotomy procedure by Biodentine 25% showed mild fibrosis (score 1) 75% showed moderate fibrosis (score 2). While Portland cement group 25% specimens showed no fibrosis (score 0) and 75% showed mild fibrosis (score 1).
After three months of pulpotomy procedure 16.66%of specimens in Biodentine group showed moderate fibrosis (score 2) and 83.3% showed severe fibrosis (score 3). While Portland cement group 25% of the specimens showed mild fibrosis (score 1) and 75% showed moderate fibrosis (score 2).
