Abstract

Management as a systematic way of managing people and things within the organization, which focuses on policy implementation, and policy formulation. It is the creation and maintenance of an internal environment in the organization for individuals and groups to perform effectively and achieve
organizational goals. It is the coordinating of all resources through the process of planning, organizing, directing and controlling to attain stated objectives. Hospital management is a continuing and dynamic application of managerial functions to provide comprehensive health care to the community. Improving patient care has become a priority for all health care providers to achieve a high degree of patient satisfaction. It is necessary for hospitals to provide the best nursing, medical administrative and supportive facilities for patients and keep track of its day-to-day activities throughout presence of high quality administrative system to achieve success and increase productivity for the hospital. The present study was conducted to examine the relationship between the quality of administrative system and health care provided to the patients in Menoufia University hospital. This aim was achieved through evaluating nurse managers, physician managers and administrative managers skills to the quality of administrative system at Menoufia University hospital, assessing staff nurses, staff physicians and administrative employees perception regarding the quality of administrative system at Menoufia University hospital and assessing the perception of the patients toward the quality of health care provided by a hospital teamwork in Menoufia University.The study was conducted at Menoufia University hospital. A co relational descriptive research design was used in conducting this study. The subjects of the study consisted of seven groups: the first group included all nurse managers (57). The second group included all physician managers (55). The third group included all administrative managers (33). The forth group included (134) staff nurses selected by Simple random sample. The fifth group included (32) physicians selected by Simple random sample. The sixth group included (199)administrative employees selected by Simple random sample. The seventh group included (75) patients selected by convenience sample.Data were collected using Managers Observational checklist Managers, Staff Questionnaire sheet, and Patients Structured interview. Managers’ observational questionnaire included three categories of quality administrative system: structure, process and outcome to evaluate nurse managers, physician managers and administrative managers’ skills to the quality of administrative system at Menoufia University hospital. Staff questionnaire sheet included three categories of quality administrative system: structure, process and outcome to assess staff nurses, staff physicians and administrative employees’ perception regarding the quality of administrative system at Menoufia University hospital.Patients Structured interview included 5 categories which are:hospital characteristics, administrative employees, physicians, nurses and supportive services to assess the perception of the patients toward the quality of health care provided by a hospital teamwork in Menoufia University.Tools were tested for validity and reliability and administered to subjects. The questionnaires were self-determined to staff nurses, staff physicians, and administrative employees and patients after explaining the purpose of the study. Data were analyzed using the appropriate tests.The results of the present study revealed that: The present study results showed that the total mean score of the administrative system quality of Menoufia University hospital structure was high quality structure. Physician managers showed the highest mean score, followed by administrative managers and then nurse managers. Regarding to process components of administrative system quality in study hospital: the present study results revealed that planning scored high quality. Nurse managers scored planning the highest,followed by physician managers and the lowest mean score was among administrative managers. Concerning to organizing item, the present study results revealed
that the total mean score of the organizing was high quality organizing. Administrative managers showed the highest mean score, followed by physician managers and then nurse managers.  Concerning to directing item, the present study results revealed that the total mean score of directing was high quality directing. Physician managers showed the highest mean score, then administrative managers. Nurse managers scored moderate quality at the directing function. Concerning to coordinating item, the present study results revealed that the total mean score of coordinating was high quality. Nurse Managers showed the highest mean score, followed by physician managers and the lowest mean score was among administrative managers. Concerning to controlling item, the present study results revealed that the total mean score of controlling was high quality controlling. Administrative managers showed the highest mean score, followed by physician managers. Nurse managers scored moderate quality at the controlling function. Concerning to leading item, the present study results revealed that the total mean score of leading was high quality leading.Administrative managers showed the highest mean score, then physician managers and followed by nurse managers. Concerning to working relationships item, the present study results revealed that the total mean score of working relationship was high quality working relationship. Administrative managers showed the highest mean score, followed by physician managers.Nurse Managers scored moderate quality at working relationships. The present study results revealed that the total mean score of the administrative system quality of outcome was high quality outcome. Administrative managers showed the highest mean score, followed by physician managers. Nursing managers scored moderate quality outcomes. The present study results revealed that the total mean score was high total quality. Administrative managers showed the highest total mean score, then physician managers and followed by nurse managers. The present study results revealed that more than half of hospital staff denoted that the administrative system quality of Menoufia university hospital structure was moderate.  There was significant difference between nurses, physicians and administrative staff regarding quality groups of process items(coordinating, controlling, leading, and working relationship).However, this significant difference was not observed regarding quality groups of process items (planning,organizing, and directing). The present study result revealed that high percentage of nursing staff agreed that the quality of coordination was high, followed by physician staff, and the lowest percentage was the administrative staff. The current study results revealed that, high percentage of physician staff agreed that the quality of controlling, was moderate, followed by nursing staff and then administrative staff. The present study results revealed that, high percentage of physician staff agreed that the quality of working relationships was moderate, followed by nursing staff and then administrative staff. The current study indicated that nurses, physicians and administrative staff scored the leading process in the hospital low, with highest percentage for the administrative staff. The present study result revealed that significant difference was observed regarding quality groups of outcome. The present study result revealed that the total mean score of the administrative system total quality in Menoufia university hospital among staff participants was moderate. Nursing staff showed the highest total mean score, followed by physician staff and the lowest mean score was among administrative staff. There was significant difference between nurses, physicians and administrative staff regarding quality groups of total administrative system scores. The study results revealed that the majority of patients denoted the quality of health services provided to them by nurses,physicians, administrative employees and supportive services were moderate quality. Nurses showed the highest percentage regarding the services provided to patients, followed by supportive personnel, physicians and then administrative employees. There was statistical significant difference between male and female patients regarding their opinions about quality of services provided by administrative employee, services provided by nurses, and total score of health care provided by MUH. The present study results revealed that although no one of managers showed low total score quality, more than two thirds of patients’ perception that the quality of services provided by MUH is moderate.  There was high significant difference between patients’ perception and each of nursing managers, physician managers, administrative managers, and total quality score administrative system. On the other hand, there was no significant difference between patients’ perception and each of nursing and physician staff.In conclusion, although no one of managers showed low total score quality, more than two thirds of patients’ perception that the quality of services provided by MUH is moderate. Therefore, the study recommended the following: Hospital administrators,accrediting agencies, insurers, and regulators should take action to provide adequate staff and resources at any time to maintain and continuously improve to improve the quality of health care and quality of administrative system. Managers should make ongoing monitoring of patient satisfaction specific to nursing and medical services regularly.Hospital managers should announce the hospital staff about job description, work regulations, and their modifications. Staff development programs are needed for managers to integrate staff in decision-making and to deal effectively with new ideas to promote satisfactory organizational climate and satisfaction of staff. Designing in-service educational programs based on management educational needs of novice nursing and medical managers and staff to maintain high quality of administrative tasks.Limitations of the study No studies were found dealing with the relationship between the administrative system as a whole and the health care provided to patients. The investigator use large, broad topic and large sample. This study contributed to this gap by exploring the relationship between the quality of administrative system and the heath care provided to patients
