Abstract

Hypospadias is a common congenital defect in which the opening of the urethra is on the underside of the penis.
A consensus on the standardized assessment of hypospadias surgery has not yet been reached.
Failed hypospadias refers to any repair that leads to complications or causes patient dissatisfaction.
The most common complications that lead to failed hypospadias repair were meatal stenosis (68% )of reterospective cases followed by urethrocutanous fistula( 23%) of cases then neourethral stricture( 13%) of cases and finally glans dehiscence( 7%) of cases.
In our study cases with flat glanular groove showed that (80.8%) of cases had meatal stenosis and (57.7% ) had urethrocutanous fistula and (42.2%) had neourethral stricture and( 3.8%) had glanular dehiscence.
Cases with shallow glanular groove showed that (87%) of cases had meatal stenosis and (51.3% ) of cases had neourethral stricture and
( 38.5%) had urethrocutanous fistula and (15.4%) had glanular dehiscence.
Irrespective of the complications leading to the failure of the primary repair, paucity of residual local tissue seems to be the greatest challenge for further surgery.
