Abstract

Diabetic macular ischemia (DMI) is a condition that didn’t receive enough attention despite of its important impact. The presence of DMI in the content of diabetic retinopathy and maculopathy affects visual acuity and changes management decisions. This study is done to evaluate the use of OCT angiography in detection of Macular Ischemia in diabetic maculopathy compared to fluorescein angiography. This prospective, non-interventional study that was done on 70 eyes ( including 50 eyes of diabetic patients who were diagnosed of macular ischemia by OCTA and 20 control eyes of healthy subjects matched for age and sex). we captured images using Optical Coherence Tomography Angiography (OCTA) using (Topcon Corp., Japan) with capturing images of the FAZ area of each eye on the level of Superficial & deep capillary layers.OCTA scan patterns of 3 × 3 mm2 and 6 × 6 mm2 centered at the macula were performed comparing with FFA images which are aligned to the OCTA images using retinal vascular landmarks and cropped to the corresponding dimensions (3 mm × 3 mm or 6 mm × 6 mm) to facilitate comparison and analysis. In this study, OCTA and FFA were able to detect macular ischemia in all patients and in the most cases macular ischemia was more evident on OCTA imaging compared to FFA. The two OCTA parameters that were used in this study to evaluate the degree of macular perfusion and ischemia (FAZ area (mm)2 and VDZ% for both SCP and DCP) were found to have a statistically significant difference between patients and control group. And also we used FAZ area (mm)2 to evaluate macular ischemia in FFA images.
