Abstract

Poly-trauma is a leading cause of death globally and usually involving young victims. Takotsubo Cardiomyopathy (TCM) is a serious clinical presentation of myocardial dysfunction characterized by acute, transient, and reversible heart failure syndrome owing to regional wall abnormalities of the ventricular myocardium associated with ECG changes and rising of myocardial biomarkers in the absence of cardiac condition causing the temporary ventricular dysfunction. The aim of the present study was to detect the prevalence of stress cardiomyopathy among polytrauma patients attending to Emergency ospital Mansoura University. The current study was a cross sectional study conducted on 500 patients who were attended to emergency department, Emergency Hospital Mansoura University, suffering from polytrauma within the period from August 2019 to August 2020.This study revealed the following results: • The majority of the studied cases were male with average age was 41 years old. •Demographic characters (age and sex) seemed to have no significant differences among stress and non-stress groups.• Around 10% of the studied cases complaining DM, HTN and hypercholesterolemia with significant rising of cardiomyopathy incidence among diabetic and hypertensive cases in comparison with cardiomyopathy free cases. •Blunt trauma was the predominant mode of trauma (85%) among the studied cases with only 15% of which were exposed to penetrating traumas.• The average value of laboratory investigations and systolic blood pressure was within the normal range of normal individuals.• Prevalence of Cardiomyopathy among the studied trauma patients was 4.4%.• The mean duration of ICU admission, ICU lenth of stay and total length of stay among the studied cases were 10.6, 6.23 and 19.67 respectively. •Higher incidence of oral hypoglycemic and insulin utilization among cardiomyopathy cases compared to cardiomyopathy free individuals. • Mode of trauma (either blunt or penetrating) had no effect in the incidence of cardiomyopathy. •SOFA and RTS scores demonstrated positive correlation with incidence of cardiomyopathy among polytrauma cases (P<0.001).• Cases with cardiomyopathy demonstrated significant reduction in Hb level and significant increase in serum creatinine level compared to cardiomyopathy free cases (P<0.001).• Cardiomyopathy group was associated marked affection of all echocardiographic parameters (Stroke volume, End diastolic volume, ejection fraction and fractional shortening) except End systolic volume.• Cardiomyopathy group was associated with marked increase in ICU admission, ICU length of stay, total length of stay and mortality rate.
