Abstract CAE is defined as localized or diffuse dilatation of coronary artery lumen exceeding the largest diameter of an adjacent normal vessel more than 1.5 fold Isolated CAE, without coronary stenosis and other heart diseases is rare. Underlying etiological causes of CAE include atherosclerosis, congenital origins, inflammatory and connective tissue diseases. CAE may result in slowed blood flow, coronary vasospasm, dissection and thrombus formation leading to increased risk of cardiac morbidity and mortality 2D-STE is a novel technique enabling more reliable and comprehensive assessment of myocardial function by obtaining the myocardial strain and strain rate in longitudinal, radial and circumferential directions .longitudinal functions deteriorate earlier than radial and circumferential functions in myocardial dysfunction Objective: The aim of this work is was to evaluate longitudinal left ventricular mechanical function in patients with coronary artery ectasia using 2D speckle tracking echocardiography Methodology: The study conducted on 60 patients who had invasive coronary angiography. Patients had been divided into 2 groups. The First group (A) included 40 patients with isolated coronary artery ectasia. The second group (B) was a control group which included 20 patients with angiographically normal coronaries. Results: Regarding Demographic data of the patients: No significant differences between group A and B regarding age, gender, HTN and smoking. Regarding Conventional 2D Echocardiography: No significant differences between the study group and control group regarding conventional TTE findings. Regarding Mitral Annular TDI Echocardiography: è wave velocities were significantly lower in group A and E/è were significantly higher in group A. So, diastolic dysfunction is significantly higher in group A than B. Regarding 2D-STE: There is significant reduction in global longitudinal strain in group A than B while no significant differences between the two groups regarding radial and circumferential strain. There is a relation between the ectatic vessel and the regional longitudinal strain of the corresponding segments. Correlation Analysis: Patients with CAE has LV systolic dysfunction, when evaluated with 2D-STE, in the form of reduction of the global longitudinal strain. Also, there is a relation between the ectatic vessel and the regional longitudinal strain of the corresponding segments. |
