Abstract

A proposal for an update by the AECC on ARDS definition has been published in 2012, which includes acute onset of hypoxemia with PaO2/FiO2 ratio less than 200 mmHg , bilateral infiltrates on chest Radiography , and no evidence of left atrial hypertension. In addition , there are no clear criteria for defining acute – leading to ambiguity regarding cases of acute or chronic hypoxemia , high interobserver variability in interpreting chest radiography , difficulties in identifying / ruling out cardiogenic or hydrostatic pulmonary edema especially in an era of using pulmonary artery catheter , and correct assessment of PaO2/FiO2 ratio owing to sensitive to changes in ventilator settings . In this Study we used ventilation strategies for ARDS based on lower end-inspiratory (plateau) airway pressure , lower tidal volumes (VT) and higher PEEP using Pressure control ventilation group (PC group) and Volume control ventilation (VC group) and Our results were : Mean age was 48.93 ± 9.91 years for PC group and majority of them (53.3%) was females . while mean age was 46.11 ± 7.94 and majority of them (53.3%) of them was males. Arterial blood gases (ABG) showed a Significant higher pH and lower PaCO2 in VC group . Mechanical parameters showed a significant higher in VT and VE in VC group . weaning and outcome in both groups was successful while 7 (46.7%) of PC group and 4 (26.7%) of VC group showed failed extubation.
