Abstract

• Primary hyperparathyroidism is more common in females and occurs in all age groups; most commonly in the age group 60-69 years. The most common presentation was asymptomatic (i.e. incidental hypercalcaemia). Therefore; screening, through serum calcium measurement, is recommended particularly in elderly females.
• Performance of IOPTH testing in intraoperatively predicting cure is superior to performance of imaging in preoperative disease localization. The performance of both US and MIBI scans are impaired by multiple factors, while IOPTH performance is impaired only with multi-gland disease.
• IOPTH testing is a valuable intraoperative adjunct that can not only improve cure rate and increase frequency of MIPs, but also it can facilitate a single scan paradigm in some patients. In such paradigm, PHPT patients who has unequivocal single gland disease on US can have MIP with IOPTH monitoring; therefore, saving cost, time and reducing radiation exposure.
• Given these benefits, development of user friendly, cost effective and fast “novel” IOPTH assay is needed.
• The study also showed that “lateral flow assay” is a suitable format for developing such assay. However, further work is needed to make it compatible with whole blood and to shorten time to result
