Abstract

Although portal vein thrombosis is a rare event among general population, diagnosis and differentiation between bland and malignant thrombus is very essential as neoplastic portal vein thrombosis gravely affects the prognosis and the subsequent treatment options.
Doppler ultrasonography usually the first noninvasive modality used to diagnose portal vein thrombosis, using color and spectral data analysis, in malignant thrombi detection of arterial waves within the thrombus could be detected.
Triphasic CT with portography is very helpful to differentiate bland from malignant thrombus. So characterization of malignant portal vein thrombosis is enabled, malignant thrombi elicit the same pattern of enhancement during the sequential phases of the multiphasic study, using maximum intensity projection (MIP) reconstruction allow better analysis of the thrombus.
Therefore, to determine the role of Doppler ultrasonography and Triphasic CT with portography in diagnosis of portal vein thrombosis and differentiation between the malignant and bland portal vein thrombi, we studied 32 patients, 21 with malignant portal vein thrombosis and 11 patients with benign portal vein thrombosis.
We found that both color Doppler ultrasonography & Triphasic CT with portography were significant in the differentiation between the neoplastic and bland portal vein thrombi.
