Abstract

Pterygium is a common external eye disease. It is wing-shaped, degenerative fibrovascular, hyper-plastic proliferative tissue actively growing from the conjunctival limbal area (secondary probably to limbal cell damage) to the cornea. Surgical removal is the treatment of choice, and the primary challenge of pterygium surgery is the prevention of recurrence This work aimed to describe the rate of pterygium recurrence after pterygium excision with bare sclera combined with intraoperative subconjunctival triamcinolone acetonide injection and limited peritomy from November 2017 to December 2019, 30 eyes of 26 patients with primary pterygium were treated by our technique This study is a prospective, non-randomized, interventional clinical study. The study included 26 patients. Out of the enrolled patients, 21 (80.8%) patients were males while 5 (19.2%) patients were females. Mean ± SE age of patients was 46.08 ±11.6 years with range 23 – 63 years Pterygia were graded preoperatively according to the grading systems described by Tan et al, 3 eyes (10.0%) were grade 2 (T2) pterygium and 27 eyes (90.0%) were grade 3 (T3) pterygium. All eyes were done with the same technique Postoperative follow-up examinations were performed at 1 week and the end of the first, third, sixth, and twelfth months after surgery In this study, recurrence occurred in only 3 eyes (10.0%) with (grade 2 (3.3%) and grade 3 (6.7%)). Elevated IOP up to 37 mmHg was noted in nine eyes (30%), which was controlled medically, and reached normal values maximally at the end of the sixth month This study concluded that the lowest recurrence rate after pterygium excision was recorded by Kheirkhah et al, (55) (8.7%) that used MMC with triamcinolone. Our study that used peritomy with triamcinolone injection, achieved a recurrence rate of (10%), followed by Rajiv Kumar et al, (7) that used conjunctival graft with triamcinolone injection. However, amniotic membrane transplantation with triamcinolone injection achieved a higher recurrence rate (16.7%) as reported by Solomon et al (61). However, this conclusion needs more studies to confirm this impression. The use of subconjunctival triamcinolone acetonide seems to be an efficient and relatively safe method to prevent the recurrence of pterygium, in addition to conjunctival peritomy was performed to expose the limbal sclera to permit stem cell harvesting, (59) which may have a role in decrease pterygium recurrence rate. Limitation of this study: A small number of patients. Absence of control group. Short follow up period.
