Abstract

Acute interstitial nephritis (AIN) is a common cause of acute kidney injury, immune-allergic reaction to numerous drugs is the most common cause of AIN.
Histamine-2 receptor antagonists (H2RAs), and more recently, proton pump inhibitors (PPIs) are used to treat gastrointestinal conditions related to acidity, such as GERD and the prevention and cure of gastro duodenal ulcers.
Proton pump inhibitors (PPIs) and H2RAs are among the most widely used medications worldwide, and often taken over-the-counter. In recent years, considerable attention has been pointed towards a wide range of adverse effects of PPIs including AIN. The majority of the observed associations are mainly based on observational studies.
Our experimental study shows an association of acute interstitial nephritis with prolonged use of a PPI (Dexlansoprazole) versus an H2RA (Nizatidine) over 16 weeks of our experimental period in male albino rats.
Immunohistochemistry (IHC) examination of the renal biopsies revealed positive renal IgG antibody in both groups. Those may show that antibody-mediated immunity may play a role in pathogenesis of drug-induced AIN in our study.
Renal biopsy confirmed the diagnosis of AIN in our study in the form of interstitial inflammation, edema and tubulitis, with no difference in the baseline kidney function tests (S.cr, S.urea, BUN and Cr.CL) or urinary output between the groups.
