Abstract

The incidence of thyroid nodules is rapidly rising, and careful risk stratification is an important element in preventing over diagnosis and excessive treatment. The role of the radiologist is critical to reduce unnecessary imaging and biopsies. One way to accomplish this is by assessing US features of thyroid nodules in a standardized fashion. However the high sensitivity of US but it has relatively low specificity in detection of thyroid cancer .As none of the US features and techniques alone is sufficient to differentiate benign from malignant tumors, a combination of these sonographic features is more successful in pointing out a subset of nodules at high risk for malignancy. ACR TIRADS provides a framework to generate structured reporting and consistently classify nodules to provide appropriate management recommendations. Implementing ACR TIRADS affects multiple aspects of the imaging workflow and also has pitfalls and challenges. Our aim was to assess ultrasound role in characterization of thyroid nodules, early detection of malignancy and reduction of unnecessary biopsy, and to confirm ACR TIRADS efficacy in accurate evaluation of thyroid nodules and its role in selection of nodules to undergo US guided FNA in order to improve the diagnostic performance and minimize unnecessary procedures.
