Abstract

Background: Acetabular fractures in the elderly are a challenging clinical problem due to both the complex nature of operative intervention, osteoporotic bone and the multiple comorbidities of the patients involved. They are associated with poor functional outcomes and high levels of morbidity and mortality. Treatment currently ranges from conservative treatment, open reduction internal fixation and variations of total hip arthroplasty,
Aim and objectives: the aim of the literature was to reveal the role of primary total hip arthroplasty for management of acetabular fracture in elderly patients according to indications, functional outcome, and complications,
Subjects and methods: This was a systematic review was consisted of ten (10) accessible articles that were found meeting the study design and the inclusion criteria, Search was being conducted using the following databases: The PUBMED, Cochrane Library, MEDLINE,
Results: Fracture union founded in 144 out of 184 cases with a percentage of 78.2%, 34 cases lost follow up or died through follow up period, 17 cases had revision surgery with revision rate 9.2%. Dislocation rate and loosing rate was 2.7%,
Conclusion: Primary THR can provide satisfactory outcomes; a prerequisite is a thorough understanding of the fracture pattern, a concise assessment of the patient risk and prognostic factors, and the right choice of implant in the fracture situation,
