Abstract

White mater hyperintensities (WMHs) of presumed vascular origin is the most frequent type of CSVDs and is defined as areas of aberrant white matter appearance with increased intensity in T2WI and FLAIR images away from areas of cortical lesions or ventricular enlargement and not related to nonvascular causes like demyelinating disease or leukodystrophy. WMHs seem to have multifactorial etiologies including endothelial dysfunction, inflammation, increased vascular permeability, blood brain disruption and venous insufficiency.The aim of this work was to study the existence and pattern of cognitive as well as balance impairments in patients with occult WMHs of presumed vascular origin.The work was a randomized cross-sectional study conducted on 44 subjects with accidently discovered WMHs (group-I) and 24 subjects with no WMHs in their MRI films (group-II) (patients with unilateral or bilateral periventricular caps were included in this group).Exclusion criteria encompassed patients with history of clinically evident stroke, cognitive impairment, manifest orthopedic problems,CADASIL / CARASIL, preexisting psychiatric or audiological disorders as well as MRI contraindications.- Written consents were taken from the patients and controls.- Research Ethic Committee of Tanta University Hospital approved the study (approval code: 32762/12/18).
