Abstract

Training of nursing interns has a considerable impact on the overall quality of health care because nurses deliver essential health care services and they spend more time with patients than other health care professionals. This is why the education of nursing interns is not just the matter of their qualification but also the matter of the quality of health care along with the enhancement of their communication skills to increase the client satisfaction and minimize the risk of conflicts between nurses and clients as well as other health care professionals. In this regard, the nursing interns should focus on the development of their practical competency which includes the formation of professional skills, critical thinking, client and nurse safety, relationship and caring and finally clinical skills (Nembhard et al, 2020).
There for, this study aimed to assess nursing interns’ needs through their practical competency self-evaluation through answering the following questions; what are nursing interns’ competency levels? , Is there a difference in nursing interns’ needs by the study setting? and What are the nursing interns’ needs based on their practical competency self-evaluation?.
This study was conducted at Menoufia University Hospital, Shebin El-Kom Teaching Hospital, TheNew Helal Hospital, Bagour Hospital, Ashmoun Hospital, Abou El Rish Hospital, Nasser Institute Hospital, Magdy Yacoub Hospital, Oncology and Burn Hospital, as well as Private Hospitals such as German Saudi Hospital, Al shifa Specialized Hospital, Ganzouri Hospital, Sherif Moktar Hospital ,Al Amal Hospital, Al Moalemin Hospital, Legislation Association Hospital where nursing interns were trained during the study.
To collect data for this study the following instrument was used: Instrument consisted of two parts Part (1): Included personal characteristics about the studysubject, this part included intern’s age, gender, name, training hospital, marital status, residency, previous graduation , and great point level (GPA).Part (2): Practical competency self-evaluation questionnaire: It was developed by Dellai, Mortari & Meretoja, (2009) and was modified by the investigator.
To assess nursing interns’ needs through their practical competency self-evaluation. It consisted of 75 statements under five Items which are professional behavior, critical thinking, injury and illness prevention, relationship and caring, clinical skills. from this instrument nursing interns were determined to be competent and had low needs or determined to be incompetent and had high needs.
Validity of study instruments:
A Bilingual group of five experts was selected to test the content and face validity of the instrument from nursing administration department (3 experts from Menoufia University and 2 experts from Benha University).
Reliability of the instruments
Reliability of nursing interns’ practical competency self-evaluation questionnaire was measured by Cronbach’s alpha and the value was (α=0.90%).
Results of the current study could be summarized as follow:
 More than four fifth of the studied nursing interns (83.4%) had age 23 years old with a mean of (23.17±0.37). The majority of them (81.6%, 74.5%, 82.15%) were female, married and had general secondary school respectively. According to their grade in academic years more than three fifth of them (71.8%) had very good grades. According to training hospitals, (64.5%) were in governmental hospitals and (35.5%) were in private hospitals. More than four fifth of them didn’t work while studying (87.9%).More than half of nursing interns (59.2) were competent with low needs, while more than two fifth of them were incompetent with high needs
 The total mean score of studied nursing interns regarding total practical competency self-evaluation items was (192.281±7.809). The highest mean score with the first ranking related to practical competency self-evaluation was professional behavior (39.524±.084) with mean percentage of (87.82%). In addition the lowest mean score with the last ranking related to practical competency self-evaluation was critical thinking (37.984±.123) with mean percentage of (54.76%).
 About 49% studied subject in governmental hospitals had high competency, 46.9% of them had moderate competency and 4.1% had poor competency. On the other hand 66.7% of studied subject in private hospitals had high competency, 31.1 of them had moderate competency and 2.2% had poor competency. Moreover, private hospitals had high competency Level (66%) than governmental hospitals (49%).
 More than half of nursing interns in governmental hospitals (52.7%) were competent, more than two fifth of studied subject (47.3%) were incompetent. While two third of nursing interns in private hospitals (71.1%) were competent, and less than one third 28.9% were incompetent. Thus nursing interns’ practical needs at governmental hospital (47.3%) more than private hospitals (28.9%).
 The highest mean score with the first ranking related to practical needs was critical thinking with mean percentage of (15.59%). In addition the lowest mean score with the last ranking related to practical needs was professional behavior with mean percentage of (12.18%).There was highly statistical significant difference between nursing interns’ level of practical competency self-evaluation and duration of working during studying, their marital status and training hospital.
 There was highly statistical significant negative correlation between nursing interns’ practical competency self-evaluations and their needs.
According to the study findings, it’s recommended through three levels:
I. At hospital level
1- Hospitals should put in their training policies nursing interns’ training principles to enhance development of nursing interns’ competency.
2- Hospitals should include in their training policies the principle of training to enhance and develop the competencies of students and nursing staff.
3- Assigning the nursing supervisors to continuously monitor the nursing interns during their work and provide them with continuous support.
4- The nursing administrators should see themselves as a role model for students and encourage them to be independent, responsible and organized.
5- Nursing administrators should allow students to participate in problem-solving and decision-making
II. At educational level:
1- Allowing nursing interns students to attend educational programs aimed at identifying their needs and improving their practical efficiency.Establishing cooperation protocols between the College of Nursing and the various hospitals to improve the competencies of then nursing interns and meet their practical needs.
III. At research level:
1- It is recommended to conduct and implement a training program to meet the needs of internship students during their training in different hospitals.
