Abstract

The use of high resolution ultrasound has helped in the last few years in the evaluation of musculoskeletal disorders with high efficiency as regard to its availability, low cost in comparison to different imaging modalities, availability of dynamic study and comparison with the other side.
MRI is currently the diagnostic method of choice in evaluation of menisci and collateral ligaments. However, the use of MRI is not only expensive, but also has some limitations including metallic implants, claustrophobia, patient’s motion artifacts, and a long examination time.
If there is a patient with history of knee trauma and clinical suspicion of meniscal injuries and/or collateral ligaments, we recommend to starts with high resolution ultrasound examination as preliminary investigation for diagnosis of collateral ligament injuries and most of meniscal injuries affecting the superficial meniscal segment. In this way; we can avoid performing the high cost MRI in a good number of patients and preserve this MRI modality for patients with borderline picture of meniscal lesions such as bucket handle tear, root tears and flipped meniscus.
Knee arthroscopy is considered the gold standard to detect the diagnostic accuracy of the different imaging modalities due to its diagnostic as well as therapeutic role in all types of suspected meniscal injuries.
A study with larger number of patients using higher resolution of MSK U/S and correlation with arthroscopy as a gold standard is needed in the future to estimate and emphasize the value of MSK U/S as a greatly promising imaging modality in evaluation of menisco-collateral lesions.
