Abstract
Overtime, breast cancer surgical management evolved from the more invasive radical mastectomy to modified radical mastectomy to breast conservative surgery. Oncoplastic breast surgery emerged after integration of oncologic principles with plastic techniques to achieve better cosmetic outcomes. Oncoplastic breast surgery consists of volume displacement and volume replacement techniques. Current volume replacement techniques are associated with donor site morbidity and scarring. In this literature we aim to assess immediate lipofilling as an alternative for volume replacement.
This study was conducted on 20 female patients admitted to surgical oncology unit, Alexandria main university hospital. The mass was excised, the abdomen was infiltrated with tumescent solution and fat harvesting was performed. After confirmation of free safety margins the cavity was approximated then processed fat was injected, then the wound was closed.
All patients were followed up for assessment of esthetic outcome and for detection of possible complications or local reccurence using clinical assessment and doppler US immediately then at 3 and 6 months interval. Esthetic outcome according to surgeons , 4 patients (20%) showed excellent results, 13 patients (65%) showed good results and 3 patients (15%) showed fair results. According to the patients using ANA score , 4 patients (20%) gave a score of 9/10 ,9 patients (45%) gave a score of 8/10,6 patients (30%) gave a score of 7/10,1 patient(5%) gave a score of 6/10. No local or systemic reccurence were encountered during follow-up period.
Immediate fat grafting is a promising technique for immediate volume replacement after BCS.
