Abstract The aim of this study was to evaluate hemostatic changes associated with therapeutic plasmaphersis with special concern to fibrinogen level risk of renal disorders on fibrinogen level changes compared to other pathologic conditions indicated for plasmaphresis. The study was conducted in nephrology unit , Ain Shams niversity Hospital. The study included 40 patients doing therapeutic plasmapheresis for any indication from May 2018 to November 2018 Patients included in our study were subdivided into 2 groups: renal group Included 20 patients undergoing therapeutic plasma exchange for renal indication , they were 4 (20%) males and 16 (80%) females. Their age ranged from 15-45 years (mean age was 29.2±6.1). non renal group : Included 20 patients undergoing therapeutic plasma exchange for non-renal indications , they were 5 (25 %) males and 15 (75%) females. Their age ranged from 15-45 years (mean age was 26.2±6.7).. All participants included in our study were subjected to • Careful history taking • Full clinical examination and monitoring during and after plasmaphresis session. • Laboratory studies included (renal function tests, cbc , INR , liver function tests and aPTT), detection of fibrinogen level. The results of the study showed: 1. The fibrinogen levels showed significant reduction following session of plasmaphresis in both groups compared to levels before session. 2. Prolongation of aPTT in all cases 3. These changes weren’t associated with clinical risk of bleeding in any group. from our study we can concluded that: fibrinogen level should be assessed in patients doing plasmapheresis session especially those indicated for invasive maneuvers or surgery following plasmapheresis sessions. No correlation between renal impairment and changes in fibrinogen levels during plasmapheresis sessions compared to non renal conditions. |
