Abstract

Acute kidney injury (AKI) is one of the most frequent organ failures encountered in intensive care units (ICU). The risk of AKI among critically ill patients varies widely across studies, usually from 30 to 60%. The aim of this thesis is to explore the clinical criteria, and predictors of outcome both mortality and renal recovery of acute kidney injury in critically ill patients in intensive care unit. Ninety (90) AKI of ICU patients during the period of this study, in whom 36 (40%) had died during the study and 54 (60%) had been discharged. Those who discharged were followed up for 3 months; 9 (16.7%) of them had been recovered completely (group 1), 14 of them (25.9%) had ESRD (group 2) and the last 31 patients (57.4%) changed to CKD (group 3). The mean age was 57.39±10.51 years with male to female ratio was 46.7/53.3 %. The majority of cases had CKD (60%), diabetes mellites (55.6%), hypertension (26.7%), heart failure (6.7%) and UTI (5.6%). Nearly half of cases had history of NSAIDs (48.5%). The median GCS was 10 ranged (7 – 14), the mean temperature 38.39 ± 0.62 oC, respiratory rate was 19.82 ± 2.44 rate/min, HR was 85.14 ± 4.95 bpm, SBP and DBP were 139.39 ± 41.68 mmHg and 84.44 ± 24.32 mmHg, respectively. The mean values of ALT, AST, Albumin, bilirubin, Serum creatinine, urea, HB, HTC, TLC and platelet count were 35.0, 35, 3.56 ± 0.54, 1.23 ± 0.29, 6.73 ± 2.06, 248.38 ± 48.81, 8.05 ± 1.06, 28.23 ± 5.59, 18.49 ± 6.18 and 173.42 ± 30.31, respectively.
