Abstract

Cicatricial alopecia is a group of disorders in which persistent loss of hair occurs due to replacement of follicles by fibrosis or hyalinized collagen. CA can be classified into PCA in which inflammation targeted directly the hair follicle (PCA) such as LPP and DLE or SCA that occurs due to accidental extension of another inflammatory process including trauma, burns, syphilis or sarcoidosis to the HF(SCA)
It can be managed medically or surgically. Surgical management is used only for subjects having lesions of stable in size provided that the inflammation that drives the hair loss has been burned out for ≥1 year. Surgical management includes hair transplantations, primer excision of involved region, flap surgery, or scar reduction with tissue expansion.
Hair transplantation has developed with years from the large punch grafts to the small mini grafts and eventually to the more refined FU graft transplantation.
Two methods are detected to harvest HFs. The 1st method is FUE, and the other oneis the harvesting HFs from scalp strip FUT.
Follicular Unit Excision denotes the surgical approach where circumferential incision of the skin surrounding the FU bundle or group of HFs to extract a full-thickness skin graft containing HFs, intradermal fat, dermal layer as well as epidermal layer.
Platelet-rich plasma (PRP) is the part of plasma fraction of the same blood that has a platelet count that exceeding the baseline. It acts as a growth factor agonist having mitogenic as well as chemotactic criteria. It contains an elevated platelet count in addition to a full complement of clotting & growth factors.
This study aimed to compare the efficacy of FUE alone versus FUE combined with PRP in management of CA
The present study was carried out on 20 patients with CA. They were randomly grouped into 2 groups, group (A) included 10 patients underwent FUE alone and group (B) included 10 patients underwent FUE+PRP.
At the end of the study the following results were reported:
PRP injection combined with the FUE provides an excellent improvement in the texture as well as the quality of the scarred tissues, but there were no significant differences in the clinical outcomes of hair restoration between both groups.
