Abstract

Accreditation is used to improve and evaluate the quality, efficiency, and effectiveness of health care organizations. Accreditation has gained worldwide attention as a system of external peer review for determining compliance with a set of standards. The accreditation process is influenced by the nursing actions as they perform all the standards operating procedures, control the quality and humanization of the service and try to provide safe care with the lowest risk to the patient. Also nurses form the basic elements in health and medical care, and their understanding and perception about accreditation standards will help to raise the level of the provided services, and to ease receiving accreditation certificate. Aim of the study To enhance nurses’ knowledge regarding accreditation standards at Tanta University Educational Hospital. Design Quasi experimental design was used to achieve the aim of present study. Such design use pre, immediate post and post 3 months to assume the actual change of nursing staff knowledge after giving of the refreshment sessions about accreditation standards. Study setting and subject The study was conducted at Tanta University Educational Hospital in all departments. It is 279 bed capacities, including the following units surgical, medical, ICUs, operating rooms (ORs), Summa Chapter VII ry 91 dialysis, incubators, neurological, urological, oncology, emergency, orthopedic, pediatric, surgery, obstetrics & gynecology, endoscopy, obesity & thinness units and bone marrow transplantation. In addition to outpatient clinics, sterilization, blood bank, laboratory, physiotherapy, mammography, elastography, magnetic resonance imaging (MRI), CT scan and X-ray departments. The study subject was selected randomly from total 350 nursing staff by simple randomly method they consisted of two categories 100 staff nurses distributed as follow 20 from medical unit, 20 from surgical unit, 20 from obstetric unit, 20 from intensive care units (ICUs) and 20 from renal dialysis unit and 20 head nurses worked in the previous mention setting at time of data collection. Tool To achieve the purpose of the study one tool was used. The tool consisted of three parts: Part I: It was included subject’s characteristics Part II: This part was assessed nurses’ perception regarding accreditation standards. It consisted of 84 items and divided into fourteen subscales about the value and impact of accreditation, patient and family rights, access and continuity of care, patient assessment, patient care, safety, support services, quality improvement and patient safety, medical records, management of information, human resources, management and leadership, medical staff and nursing services. Part III: This part was assessed nurses’ knowledge about accreditation standards. It included 30 questions; in form of multiple Summachoice (10 questions) and true and false (20 questions). Nurses were divided into four groups. Each group was attended three sessions, the duration of each session was (1 hr.) for three consecutive days. Main result of the study - Regarding the age around half (62% and 50%) of staff and head nurses aged between 19-<25 - There were statistically significant differences between the staff nurses and head nurses regarding education level, years of experience, and number of training courses at (p< 0.05). - There were statistically significant differences at (P<0.05) between head nurses’ mean perception scores pre, immediate post and 3 months post implementation of refreshment sessions regarding all dimensions except for access and continuity of care and nursing services dimensions. - There was a statistically significant difference between head nurses’ and staff nurses’ levels of perception of accreditation standards pre, immediate post, and three months post implementation of refreshment sessions at p value 0.0001*. - Only more than one third (38.3%) of the nurses had high perception level regarding accreditation standards at pre implementation of refreshment session this result improved to be (100%and 92.9%) at immediate post and 3 months post implementation of refreshment sessions respectively. - The staff nurses’ knowledge mean scores were 19.89±2.9 5 at pre-implementation that improved to be 28.59±2.49 and 25.59±5.04 at immediate post and three months post implementation of refreshment session respectively. - The head nurses’ knowledge mean scores were 22.00±2.85 at the pre-implementation that improved to be 28.60±1.57 and 27.69±4.32 at immediate post and three months post implementation of refreshment session respectively. - The staff nurses and head nurses’ knowledge were improved immediately post implementation of refreshment session regarding all items. - There were no significant differences between staff nurses and head nurses’ responses regarding all items of accreditation criteria. - There were no significant difference relation among staff nurses and their characteristic data except for change immediate post than pre implementation there was significant difference at (P<0.05) regarding years of experience. - There were significant difference at (P<0.05) regarding marital status and attending training courses (0.042 and 0.033) respectively for change immediate post than pre implementation of refreshment sessions. Based on result of study it was recommended to establish periodic orientation programs for refreshing nurses’ knowledge about accreditation standards by hospital administration. The head nurses conduct periodical evaluation of staff nurses performance in demonstration of accreditation standards in the hospital.
