Abstract

This study was conducted to evaluate and compare the post-operative pain after using XP-Endo Finisher file versus ultrasonic activated irrigation versus dynamic manual irrigation in the final irrigation protocol in single visit endodontic treatment of symptomatic irreversible pulpitis.
Fifty six patients with symptomatic irreversible pulpitis, mandibular molars teeth were endodontically treated in a single visit. The patients were divided into 3 groups, each consisted of 18 patients. Instrumentation was done using Fanta rotary file system. Irrigation was done using 2.5% NaOCl. Final irrigation step was either using XP-Endo Finisher f ultrasonic activated irrigation or manual dynamic agitation. Postoperative pain was assessed using a Numerical Pain Rating Scale at 6, 24, 48hours. All demographic data, clinical findings and numerical rating scale scores obtained from the patients were statistically analyzed. When comparing the median of NRS scores in the three groups, there was no statistically significant difference between them after 6, 24, 48 hours postoperatively.
Conclusions
Within the limitations of this study, it could be concluded that:
1. Postoperative pain intensity and incidence were similar when using XP-Endo Finisher or ultrasonic activated irrigation or manual dynamic irrigation in the final irrigation step in one-visit endodontic treatment of multiple canaled teeth with symptomatic irreversible pulpitis.
2. Postoperative pain decreased gradually over time regardless of the irrigation activation technique used.
Recommendations
Further studies are required to:
1. Perform bacterial cultural analysis following single visit root canal preparation of symptomatic irreversible pulpitis after final irrigation step with XP-Endo Finisher and ultrasonic irrigation, and before obturation.
2. Further randomized controlled trials with larger sample sizes for endodontically treated symptomatic irreversible pulpitis with final step activation of irrigation with XP-Endo Finisher and ultrasonic irrigation are mandatory to confirm these results.
