Abstract

Hundred thirty five adolescents with T1DM (mean age ± SD of 14.86 ±1.71 years; range 12 -18) were compared to 135 age and gender matched healthy adolescents. Adolescents with T1DM had significantly higher NVC alterations than controls. The most common NVC alterations among the studied adolescents with T1DM were tortuous capillaries (75%), cross linked capillaries (69%), giant capillaries (30%) and avascular areas (25%). Two adolescents with T1DM had G1 NVC (1.5%), thirteen had G2 NVC (9.6%), 40 had G3 NVC (29.6%) and 80 had G4 NVC (59.3%).
A significant positive relation was found between NVC grading and each of diabetes duration, TCSS, HbA1C%, total cholesterol and LDL. In addition, adolescents with T1DM with clinical and subclinical diabetic neuropathy, retinopathy, and nephropathy had significantly higher NVC changes than those without these complications.
Multivariate logistic regression analysis for predictors of adolescents with T1DM with neuropathy showed that insulin dose, NVC changes and TCSS were the most important predictors of diabetic neuropathy. In addition, multivariate logistic regression analysis for predictors of diabetic nephropathy revealed that total daily insulin dose and NVC changes were the most important predictors of diabetic nephropathy
