Abstract

With advances in medical therapy and life expectancy, reoperation to replace dysfunctional mechanical heart valve prosthesis is an increasingly common procedure. There have been gradual decreases in perioperative risk for redo valve surgery over the past 2 decades, likely due to increased surgical experience, better myocardial protection, and improved patient management. However, mortality rates remain higher than first-time valve replacement surgery. Many factors result in dysfunction of mechanical valves: thrombosis, pannus, paravalvular leak as well as endocarditis. Our work was a observational case –control retrospective study and it was designed to detect and analyze the risk factors of prosthetic mechanical mitral valve thrombosis and to discuss the surgical management. It was two center observational case control retrospective study was conducted at Cardiothoracic Surgery Department-Mansoura University Hospital and Nasser Institute for Research and Treatment. 60 patients were divided into two equal groups, group A (case group) included 30 patient and group B (control group) included 30 patients. The study was designed to detect and analyze the risk factors of prosthetic mechanical mitral valve thrombosis and to discuss its surgical management. In our work it was observed that many factors contribute in the thrombosis process of the mechanical mitral valve. As regard to the demographic data, the body weight of case group showed statistically significant increase more than control group with P value 0.001. There was no statistically significant difference in age and gender but the pregnant female were higher in case group with P value 0.033 than the control group.
