Abstract

The aim of the study: is to assess the central venous-arterial carbon dioxide difference as a goal in goal directed therapy of sepsis resuscitation in comparison to central venous oxygen saturation and to test for their prognostic values.
Patients and methods: A Prospective randomized study included 60 critically ill patients who will fulfilled the diagnostic criteria for sepsis and septic shock were randomly assigned to one of two group each one included 30 patients. Each group received early goal directed therapy resuscitation. group A (central venous-arterial co2 gap) and group B (central venous oxygen saturation) as goals for goal directed therapy respectively. Central venous oxygen saturation, central venous-arterial CO2 gap were measured simultaneously at (T0), (T6), (T12), (T24) at the time of study inclusion. The non-surviving patients in group A showed persistently high CO2 gap value in comparison to the surviving patients. In group B, the non-surviving patients also showed progressive decrease in ScvO2 values in comparison to the surviving patients who showed a progressive improvement in ScvO2 values.
Conclusion; Central veno-arterial PCO2 gap has a good predictive value and can be used as a goal for early goal directed therapy.
