Abstract Background: Diabetes mellitus (DM) is one of the most common diseases in the world and is increased globally. Cardiovascular complications, such as left ventricular dysfunction is a major cause of death in patients with type II DM. Alteration in cardiac function can be non-invasively assessed via echocardiography. The early recognition of subclinical left ventricular dysfunction (systolic and diastolic) can prevent the symptomatic heart failure in diabetic patients. Aim of work: The aim of the study was to determine the prevalence of left ventricular dysfunction in asymptomatic type II DM patients. Settings and Design: This was a prospective case control study, which was conducted in outpatient clinics of cardiology & endocrinology and inpatient cardiology department at Fayoum University Hospital. Subjects and Methods: Seventy patients diagnosed with Type 2 DM without any feature of coronary arterial disease (CAD), hypertension, nephropathy and respiratory illness were enrolled in this study and compared with thirty age-matched healthy controls. Echocardiographic assessment was done in all subjects to evaluate the left ventricular diastolic and systolic functions. Results: Seventy patients, (23 males and 47 females) and thirty healthy controls (13 males and 17 females) were enrolled. Mean age of patients was 49.86± 10.85 years and 51.07± 10.78 years for controls. Mean body mass index was 28.51± 2.25 kg/m2 in patients and 28.87± 2.42 kg/m2 in controls. Mean ejection fraction was 62.31% ± 9.75% and 66.27% ± 9.63% in patients and controls, respectively (P = 0.065). Six (8.6%) patients had ejection fraction less than 50% compared to none (0%) in controls (P = 0.174). Impaired diastolic function was found in 70% of patients compared to 40% of controls (P= 0.007). Conclusion: Diabetic patients have a high prevalence of left ventricular dysfunction even in the absence of cardiac symptoms. Diastolic dysfunction is more common in diabetic patients when compared with normal healthy population. Systolic dysfunction progresses with age of the diabetic patient. Echocardiography is a simple noninvasive cost effective test for detecting cardiac dysfunction in Type 2 DM patients and should be applied to detect early Left ventricular (LV) dysfunction so that corrective measures may be initiated early and cardiac functions may be preserved for long.
