Abstract

Introduction : Radial longitudinal deficiency (RLD) is the most common congenital longitudinal deficiency at birth and represents a wide spectrum of upper extremity anomalies, from mild thumb hypoplasia to absent radius. Radial dysplasia may be isolated or associated with an array of systemic anomalies that should be familiar to hand surgeons. The Aim of this study : A retrospective and prospective study designed to evaluate the potential growth of the ulna following centralization of children with radial longitudinal deficiency presented to Hand surgery unit- Mansoura University Hospital from January 2014 to January 2021. Materials and methods : After approval from local Institutional Review Board (IRB) and obtaining written consents from parents of enrolled children, in the period from January 2014 to January 2021, a retrospective and prospective case series study was done including 20 RLD limbs in 16 children presented to the Hand and Upper limb Surgery unit managed by centralization of the hand over the distal ulna. Results: Using non-notched procedure through circumferential rotational flap was safely used for centralization in this study preserving the growth potential of the distal ulna without memorable complications attributed to the flap, leaving a smooth satisfactory contoured wrist. Outcome measures show that patients maintain useful active motion, and along with their parents, are satisfied with both the appearance and function.
