Abstract

FP is a devastating condition that affects the patient’s quality of life and is considered a challenge for surgical repair. Peripheral FN injuries and transection is a common condition either for one or more branches of FN. Frontal and zygomatic branches which supply orbital complex musculature concerned with great importance due to the control of eye closure coordination.
The current study was conducted on two phases phase one to assess the function of muscle clinically to assess the muscle fiber of OOM after it re enervated by this new technique after FN transection. We choosed that the method based on developing idea named supercharge concept as HFN to overcome the traditional surgical repair techniques drawbacks.
The current research was carried on six goats where the right side acted as experimental while the left acted as control without any intervention. The right Facial nerve (FN) was intentionally transected and immediately repaired using an ipsilateral end to end hemi hypoglossal nerve
(HHN) neurorrhaphy to the distal facial trunk and end to side to its proximal end ”supercharge”. One year later animals were anaesthetized and samples were bilaterally harvested from (OOM). Both groups were histologically and clinically assessed and compared.
Assessment scheduled for:
1- Clinical assessment either air flow test or cotton touch test immediately preoperative, one month postoperative, three months post-operative ,six months postoperative and finally one year post-operative.
2- Histological assessment by light microscopy and electron microscopy on biopsy of OOMs one year after surgury
The results of the clinical assessment of the blinking score with the air flow test or with cotton bud touch reflex revealed that there is no significant difference between the immediate preoperative results and the six months postoperative.
The results of histological assessment statistically non-significant difference between the control group and supercharge group (SG) in general arctikture of muscle fiber and statistically significant in the mean area of collagen fibers and cross- sectional area regarding it does not affect the function of muscle.
Thus we reached a final opinion that the developing supercharge concept is reliable and valid and HFN anastomosis is very helpful to restore the muscle function , but we recommend to doing negative control group in which the FN or the ZFNB is transected and left for spontaneous total fibrosis and applying the technique to humans level.
