Abstract

Psoriasis is a common chronic inflammatory skin disease. Onset may occur at any age but peaks in the second and third decades of life. The course of the disease is characterized by relapses and remissions but the condition tends to persist throughout life Balneotherapy is an effective, well tolerated, complementary approach in the treatment of several pathologies mainly those related to chronic inflammation such as psoriasis (Nasermoaddeli and Kagamimori 2005) Our study aimed to compare the efficacy and safety of climatotherapy versus topical combination of steroids with salicylic acid for treatment of psoriasis.The present study was carried out during the period between February 2018 to February 2019. The present study included 30 patients with psoriasis vulgaris. The age of patients ranged from 9 to 60 years. It included 17 male patients and 13 female patients. There were randomly divided into two treatment groups: group I treated with climatotherapy and group II treated with topical combination of steroids with salicylic acid, they are recruited from outpatient dermatology clinic of Hurghada general hospital in calloporation with Dermatology, Venereology and Andrology department, Assiut University Hospital.All patients were subjected to complete history, general and local examination. The efficacy of treatment was evaluated by digital photographs, PASI score and skindex 16 questionnaire.in biopsy was performed for histopathological examination and immunohistochemistry of IL22 expression to all studied patients.No statistically significant differences were found between two groups of patients regarding age, gender, occupation or marital status (P value >0.05).There was statistical significance decrease in PASI score before and after treatment in both groups (P value 0.001).There is statistically significant difference between histopathalogical changes before and after treatment in group I and group II (P value <0.05).There was statistically significant positive correlation between PASI score and histopathalogical score of Trozak in both groups (P value <0.001 for group I and 0.002 for group II).Immunohistochemical expression of IL-22 in epidermis and dermis was significantly decreased after treatment in both groups but it was significantly decreased in group I more than in group II. There were statistically significant decrease in immunoreactivity score of epidermis and dermis in group I after climatotherapy treatment compared to group II (topical combination of steroids with salicylic acid) (P value 0.000). There was statistically significant positive correlation between immunoreactivity score of epidermis and histopathalogical score in both groups (P value for group I 0.001 and for group II was<0.001).
