Abstract Summary Background: Cataract is the most common cause of diminution of vision among old people . Phacoemulsification is a modern safe surgery for treatment of cataract which give very great results and achieve big improvement in visual acuity in patients. Despite ever-evolving technology, residual refractive errors still occur which can cause anisometropia or is a source of patient dissatisfaction due to unmet expectations . Aim of the study: To determine the incidence of refractive surprise after phacoemulsification. Material and methods:. Tracking 50 eyes of patients who underwent uncomplicated cataract surgery. Result: 50 eyes of uncomplicated phaco cases were included , 5 of them showed postoperative refractive surprise (10%), three patients have refractive error between -1.5 and +1.5 , the incidence of postoperative refractive surprise washigher among patients with long axial length than patients with short axial length , higher among patients more than 70 years old (22.2%) followed by patients ≤ 50 years old (15.4%) , higher among patients with myopic fundus than patients with normal fundus (15.0%) and (6.7%), respectively. Conclusion: The best way to manage refractive surpriseis to prevent it by Accurate biometry, A-constantoptimization, intraocular lens (IOL) formula selectionand avoiding wrong lens implantation. Options of treatment include spectacles and refractive surgery and lens exchange according to degree of error. Key words: Refractive error , refractive surprise, phacoemulsification. |
