Abstract

Acne vulgaris is a common and chronic inflammatory skin disease of pilosebaceous unit. Obesity is one of the biggest problems in western lifestyle but nowadays, the problem is increasing even in low and middle-income countries. Body Mass Index (BMI) is used to accurately measure obesity.
This present study aimed to correlate between the overweight/ Obesity and severity of acne, and to find if there is a link between BMI and the amount of expression of RAR-γ in skin tissues of acne patients, and the subsequent supposed effect on therapy response by isortretinoin.
The study was a cross-sectional, analytical study that had been conducted from November 2019 to April 2021 in the Dermatology outpatient clinic at Beni-Suef University Hospital after the approval of REC on 60 patients 23 males (38.3%) and 37 females (61.7%) with age ranged from (13) to (30) years old with an average age of (21.47 ±4.39) years old , their MI was ranged from (21.20) to (46.31) with an average BMI of (30.13 ±5.81) kg/m2. They were classified according to their BMI into three subgroups [group (I) 12 normal weight individuals, group (II) 21 overweight individuals and group (III) 27 obese individuals]. All participants underwent complete history taking. Measurement of RAR-γ tissue expression was conducted by human RAR-γ ELISA kit according to manufacture instructions.
Our data analyses reveled that Serum glucose level, insulin level and Homa-IR score (for estimating insulin resistance) were significantly highest among obese individuals as compared to both normal weight and overweight. There was non-statistically significant difference between normal weight and overweight individuals. The RAR-γ expression was lowest among obese individuals. The expression level was significantly lower in obese as compared with normal weight individuals, and significantly lower in overweight compared to normal weight individuals. There was non-statistically significant difference in RAR-γ expression level between overweight and obese individuals. We obtained a statistically significant linear negative correlation between RAR-γ expression and the Serum glucose level (r= -0.519, p=0.001), Insulin (r= -0.485, p=0.001), Homa-IR (r= -0.485, p=0.001), and BMI (r= -0.470, p=0.001) in all studied subjects collectively.
We concluded that; tissue expression levels of RAR-γ was down regulated in acne patients in relation to their BMI. There was no association between BMI and severity of acne in the current study. Larger studies are needed to confirm these findings. Better characterization of role of RAR-γ could constitute a therapeutic avenue to the treatment of Acne Valgaris.
