Abstract

Thyroidectomy is considered one of the most frequently performed operations in general surgery. It involves meticulous devascularization of the thyroid gland,which has one of the richest blood supplies among the organs. Use of energy-based devices for thyroid surgery is recognized as safe and effective and is associated with intraoperative advantages, post-operative outcomes and cost-effectiveness. The challenge is to decide the device that is appropriate under the specific circumstances, and the choice is based on the surgeon’s preference or experience The current study was conducted to assess safety and efficacy of ligasure compared to the conventional vessel ligation in thyroidectomy. A total of 80 patients scheduled for thyroidectomy were enrolled in the study. Those patients were randomly subdivided into groups; group I included patients who underwent conventional vessel ligation (n= 40 patient) and group II included patients who underwent ligasure vessel sealing (n=40 patients)Both groups had insignificant differences regarding age and sex . The main findings of the current study were that ligasure group had significantly lower operative time and intraoperative and postoperative blood loss. Also, ligasure group had significantly shorter hospital stay , although we found that mean cost of ligasure vessel sealing was significantly higher in comparison to conventional vessel ligation . but there was insignificant difference as regard overall recorded complications . The most frequent complication was transient hypocalcaemia. In conclusion, although Ligasure was significantly advantageous over conventional technique in reducing operation time , intra operative blood loss , postoperative drainage , parathyroid injury and hospital stay .But the overall cost of surgery was significantly higher in Ligasure group.
