Abstract

Background: Preeclampsia is a multisystem disorder of pregnancy which is a major cause of maternal and fetal morbidity and mortality worldwide. The cardinal clinical features of the condition are hypertension and proteinuria occurring after 20 weeks gestation in women who were not previously known to be hypertensive. The uterine artery Doppler has potential rule for screening for preeclampsia.
Aim of the Work: The aim of the work was to evaluate the role of uterine artery Doppler in the second trimester as a predictor of preeclampsia in high risk pregnancy.
Patients and Methods: this prospective study was carried out in Ultrasound special care unit of the fetus, Ain Shams University. This study included 80 pregnant females in the second trimester (between 16-25 weeks gestation) with high risk factors for developing preeclampsia , uterine artery Doppler ultrasound was done between 16-25 weeks of pregnancy for these patients. The Doppler indices generated automatically from the machine , the Pulsatility Index (PI), Resistance Index (RI) ,S/D ratio and presence or absence of early diastolic notch were recorded. the patients were followed up in the third trimester for blood pressure measurement and diagnosis of PE.
Results: out of 80 cases included in our study, 19 cases (23.8%) developed PE. 6 cases had a previous history of PE and 11 cases were diabetic. There was statically significant correlation between DM and preeclmpsia (P-value < 0.05) , while there is no statically significant correlation between history of previous preeclampsia, PG and multipara women (P-value > 0.05) .when uterine artery Doppler was done, it was found that elevated indices (RI, PI,S/D ratio) and presence of early diastolic notch were significantly associated with developing preeclampsia later in pregnancy (P-value < 0.05). Regarding our study, when RI was used , sensitivity and specifity were 84.2% , 91.80% . when PI was used sensitivity , specifity were 94.7 % , 96.72% . when S/D ratio was used sensitivity, specefity were 89.47 % , 93.44% .
Conclusion: Our study revealed that we can use uterine artery Doppler as a reliable method to predict preeclampsia.
