Abstract

The incidence of various diseases increases with obesity, especially cardiovascular diseases, diabetes mellitus type two, obstructive sleep apnea, many types of cancer, osteoarthritis, and depression. Aim of this study : to compare between the effect of using CPAP or incentive spirometer preoperatively on post operative pulmonary complications in morbidly obese patients underwent laparoscopic bariatric surgeries. Patients and methods : It included 52 morbidly obese patients underwent general anesthesia for laparoscopic bariatric surgery over two years duration from april 2020 to February 2022. Methods : Fifty-two Patients were randomly allocated using computer generated Program and closed envelop method into two groups. group CPAP (26 patients)patientswere subjected to continuous airway pressure 10 cmH2O for 10 minutes every 2 hours on the day before operation till the time of surgery, while group IS (26 patients)patientswere instructed to use spirometry one day before operation by inhaling as slowly and deeply as possible in a set of 10 times and repeat the process every 2hours till the time of surgery. Results: the effect of active treatment by incentive spirometer is better than the effect of passive by CPAP. By analyzing and processing the results obtained our study we concluded that : Pre operative application of CPAP or incentive spirometer lower the incidence of post operative pulmonary complications, improve intra operative lung mechanics and post operative oxygenation. But the effect of active treatment by incentive spirometer is better than the effect of passive by CPAP
