Abstract

Ameloblastoma (AM) is the most frequently encountered tumor arising from odontogenic epithelium. It is characterized by a benign nature, also it has an aggressive and invasive behavior and high rate of recurrence. Matrix metalloproteinase-1 (MMP1) is a member of the matrix metalloproteinases family, and its aberrant expression is implicated in tumor invasion and metastasis. The aim of this study was to compare the immunoexpression of MMP-1 in different types of AMs and to correlate the clinical, radiographic and histological features of AMs in relation to MMP-1 expression. Material and Methods: The lesion was immunohistochemically examined for presence of MMP-1 using the labeled Strept-Avidin-Biotin-Peroxidase complex system (LSAB) and evaluated using light microscope. Statistical analysis was performed using SPSS version 26.0. P value (< 0.05) was considered statistically significant. Results : Most of the studied cases were in young age (64%). The cases were exhibited no gender predilection. The mandible was the most affected site in AM. Conventional cases were larger than unicystic cases. Also, all conventional cases revealed multilocular radiolucency. The most common histological variant was the follicular form. The mural type was the most frequent unicystic type. The statistical analysis revealed a significant difference between conventional and unicystic AM cases regarding age, size, x-ray. Meanwhile, there was a non significant difference between them regarding gender and site. There was a significant difference between old & young age cases, as well as between unilocular & multilocular cases and between small& large cases regarding expression to MMP-1. There was a significant difference between both conventional and unicystic lesions in relation to MMP-1. While, there was a non significant difference between follicular and plexiform types in relation to MMP-1. The expression of MMP-1 was more prominent in conventional AM than in unicystic type. Conclusion : In the light of the findings of the present study, we can conclude that : 1. Demographic data of AM., revealed that it is most frequently occurred in the third decade of life, no gender predilection, the posterior mandible is the most frequently affected, mostly a large tumor size and appeares as unilocular or multilocular radiolucent lesion. 2. Stromal cells of AM are involved with the neoplastic cells in the regulation of tumor progression and expansion due to their role in degradation of matrix components. 3. Conventional AM is more aggressive than unicystic type as conventional AM revealed a higher expression of MMP-1.
